Office Policies
 Appointments:A missed appointment is a loss to several people: the child, the doctor and staff, and another child who could have used this valuable time reserved especially for you. Please inquire about our cancellation policy. Insurance: Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary greatly, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of individual insurance contracts. For all procedures, including professional cleaning and exam appointments (6 month check up), our office will collect a percentage of the total cost of the procedure. As a service to our patients, we will file insurance claims for you* and allow them 30 days to render payment. Upon the receipt of insurance payment, a credit will be applied to your account. Any remaining balance will then be communicated to you through the form of a statement. After 60 days, you are responsible for the entire balance, paid in full if insurance has not responded or has denied your claim. If you have a pre-treatment estimate that your insurance company has sent you and you would like us to honor those fees, we require that you have it in hand at your appointment so that we can record the data. If you have any questions, our courteous staff is always available to answer them. Financial arrangements: Our office staff will assist you in the breakdown of all applicable fees, and will inform you of all costs before treatment is administered.* Charges are payable at the time of treatment. Please ask a member of our staff, and we will review our payment options to accommodate your financial needs. We do accept MasterCard, Visa, cash, personal checks, and CareCredit. * Estimated costs for professional cleaning and exam (6 month check up) appointments will be given upon request. |
